PIAA Physical Information
All physicals for sports participation must be completed on the approved PIAA physical forms. These can be found online, in the Athletic Director’s office, or in the main offices of both the middle school and high school. We cannot accept a yearly check-up, drivers permit physical, etc.
The physical form consists of the following sections:
1) Emergency Contact Form – name, address, phone, emergency contacts, etc.2) Certification by Parent/Guardian3) Understanding the Risk of Concussion and Traumatic Brain Injury4) Understanding of Sudden Cardiac Arrest Symptoms and Warning Signs5) Health History
6) Certification by Medical Examiner
7) Recertification by Parent/Guardian Form
8) Recertification by Medical Examiner
9) Wrestling Form

PRE-PARTICIPATION PHYSICAL
The first physical of the school year must include Sections 1-6 filled out completely and signed by a physician.
RECERTIFICATION FORMS (Sections 7 & 8)
For recertification, we will also include another Emergency Contact form (Section 1) in case any changes have to be made.
If you answer "yes" to any of the questions in Section 7, you must have Section 8 signed by the physician who performed your original physical. If it was done here at the high school, you must attend the physicals at the high school for a recertification exam.
SECTION 9
This section is used only for wrestling and will be made available to the wrestlers at their preseason weight certifications.
Lastly, the state is now mandating that all male athletes be screened for hernia. These medical exams will take place in separate, isolated areas to respect the athletes’ privacy. PLEASE NOTE: The medical examiners consist of both male and female physicians/nurse practitioners.
