    Presenting Required Documents

    After completing the online registration, you will receive an email advising you of your child's school assignment and providing a link to the appointment calendar to present the following documents:

    • Original birth/adoption certificate
    • Up-to-date immunization records
    • Proof of residency in the form of a current deed or lease and 2 utility bills
    • Custody orders and/or other legal documents if a court appointed foster parent (if applicable)
    Please do not schedule an appointment until after you complete the online registration.
