Student Registration

In order to successfully register your child at the Pottsgrove School District, you must provide all of the required documentation.

If you are transferring from another school district, the prior school district will NOT automatically send Pottsgrove documents. It is the responsibility of the parent to obtain and send in the necessary documents to register their child with Pottsgrove.

DISTRICT RESIDENT

Please be advised that only a parent or court-appointed legal guardian living in the school district and at the same residence of the student may register a student. Parents/ legal guardians are legally obligated to provide accurate information during the registration process.  A girlfriend or a boyfriend is NOT a valid parent or legal guardian and can ONLY be added to the emergency contact list.

1. Proof of the Child's Age (any one of the following is acceptable)

• Birth certificate (preferred) if you don't have one click here: Application for a Birth Certificate
• Notarized copy of the birth certificate
• Baptismal certificate
• Copy of the record of baptism - notarized or duly certified and showing the date of birth
• Notarized statement from the parents or another relative indicating the date of birth
• Valid passport: Apply for a Child's U.S. Passport
• Prior school record indicating the date of birth

2. Up-to-date Immunization Records or Immunization Exemption Statement Form

The Pennsylvania Department of Health’s immunization regulations require parents or guardians of students enrolled in grades K-12 to have their children immunized against various diseases to protect the health and safety of all students. 

Requirements: 

• School Vaccination Information for Parents (English) 
• School Vaccination Information for Parents (Spanish)
• If applicable: Medical or Religious Exemption Form

Please make sure to ask the doctor to review your child’s Immunization record to verify your child is up to date on all required immunizations. If your child's immunizations are not up to date or if they are missing any of the required immunizations, they must have a scheduled doctor’s appointment or they may not begin school until the immunizations have been administered and the updated record is provided.

If Applicable: Tuberculosis Testing Requirement - When relocating or returning from Out of Country to Pennsylvania:

It is a requirement of the State of Pennsylvania to complete TB testing after arriving in PA. Please see the link below. We follow the Montgomery County Department of Health Regulations regarding TB. If you received BCG (Bacille Calmette-Guérin) vaccine as a child or were born in or frequently travel to a country with high rates of Tuberculosis disease you will need to have the blood test called IGRA (interferon-gamma release assay) administered. TB testing is required regardless of BCG Immunization status.

3. Proof of Residency (two proofs required)

Acceptable 1st Proof of Residency (one of the following):

• Deed - All pages and notarized signatures required.  If you need a copy: Click here to request a copy of your Deed in Montgomery County

• For families purchasing a new home or new home build ONLY!  Please provide your signed Agreement of Sale or Purchase Agreement (All pages required) and the agreement must indicate a set settlement date to be reviewed by the registrar prior to registering. Settlement on your new home must occur within 60 days of the student's start date per Pottsgrove SD Board Policy. Soon after closing on your new home please provide the deed including all pages and signatures and current utility bill or utility activation to studentregistration@pgsd.org to update your former address to the new address.

• Property tax bill - must be current and up to date
• Current Fully Executed Signed Lease or Rental Agreement - All pages required and must be signed by the Lessee (Renter) and Lessor (Landlord or Property Owner / Manager). 

• If you do not have your current Lease or Rental Agreement, please reach out to your Leasing Office or Landlord to request a copy of your Lease Agreement.  HUD Acknowledgements are not acceptable as a lease agreement.

Acceptable 2nd Proof of Residency (one of the following):

• Current utility bill (gas, water, electric, phone)  - must be the current, full statement or utility activation email or letter for new residents with the address and date of activation.  You can reach out to the utility company and request an activation letter.  We do not accept a screenshot of the bill in the envelope. 
• Current credit card bill - must be the full statement.  We do not accept a picture of the bill in the envelope.
• Current Mortgage Statement - must be the current, full statement
• Vehicle registration with the current address. - Click here to update your PA Vehicle Registration

• Driver's license or Dept. of Transportation identification card with the current address. Click here to update your PA Driver's License

• A piece of mail sent by the federal or state government such as benefits information, voter registration, or tax documents

4. Academic Records:

• Current Report Card
• Transcripts
• 504 Plan
• Special Education Records: IEP or GIEP

5. Court Ordered Legal Documentation Pertaining to Custody (if applicable):

• Court Ordered Child Custody Agreement
• Court Ordered Protection from Abuse
• Foster Placement Letter from an Agency
• Legal Adoption Certificate / Decree
• Power of Attorney Legal Documentation

The Pottsgrove School District requires verification of the residence of all students residing in the district. This would also include any address changes that might take place throughout the school year.

The Multiple Occupancy Form only applies to families living with a district resident where the proof of residence is not in your name. Both the parent / legal guardian and student must live in the same residence together to register. Please be advised that you are legally obligated to provide accurate information.  Any Multiple Occupancy residents will be required to have a new form signed each school year. 

When registering your child, you will need the items from the list in “What do I need to Register” regarding Proof of Age and Immunizations for your child, as well as the following items:

1. Proof of Residency (two proofs required) for the Homeowner or the Lessee of the person renting the property.  You will also need to provide one item from the list in the second proof of residency in your name with that address on it (i.e. credit card bill, driver’s license/update or vehicle registration).

2. Multiple Occupancy Form (two pages) signed and notarized

If you are residing with a current Pottsgrove School District resident, you will need to fill out the Multiple Occupancy Paperwork listed below. These documents must be filled out by both the parent/legal guardian as well as the resident of the Pottsgrove School District. We will still need a copy of the verification from the list above to show residency in the Pottsgrove School District.

The district may conduct random observations to follow up on residency compliance throughout the school year. If it is determined that the conditions of the Multiply Occupancy Affidavit are not being met, the Parent/Legal Guardian and the Resident will be liable to reimburse the Pottsgrove School District for, but not limited to, tuition, legal costs, court costs and administrative costs.
